Media Reserve Request Form

Forms are available here for faculty to send the library lists of items that need to be placed on reserve for their courses.
These forms should be used only for items the library already owns or for items the library needs to order. Personal copies should be brought to the library.
Select the library where the reserve materials will be made available for your students. If the item currently belongs to another Vanderbilt library, the library receiving your request will take the necessary steps to retrieve the material from that library if the item requested is a circulating item.
Personal Materials
- Books: Personal copies of books may be placed on reserve. Staff will label and barcode all personal materials. Although staff take all reasonable precautions, personal materials on reserve are at some risk. Do not place valuable or irreplaceable items on reserve. Books should be labeled with the name of the owner.
- File Folders (photocopies, articles, tests, overheads, etc.): Instructors are responsible for complying with copyright laws. If you wish to add material in a series on a continued basis throughout the term, initiate this procedure by submitting the first in the series in a binder or notebook.
